Ischiorectal fossa is a wedge shaped structure that has an apex and a base, where the apex is placed upwards and the base is placed downwards. This is a fat filled structure which helps in defecation by dilating the anal canal. As this structure is scantly vascularised the rate of spread of infection seems to be rapid. Out of 3 cases of anorectal abscesses 1 would be an ischiorectal abscess. This happens due to the outburst of infection of anorectal abscess through the external sphincter, while the anorectal abscess occurs due to the infection of cryptoglandular epithelium and also the anal canal.
